50 Page Triplicate Pad - This is a combined Testing register & Chain of Custody form for both Urine and Oral Fluid combined to conduct testing in accordance with new Australian Standards requirements AS4308:2023 and the current AS4760:2019.
Document your tests using the Testing register. This pad of 50 forms with triplicate carbonless copies helps your Collector:
- Record the donor's identification
- Document the breath alcohol screening and confirmation results,
- Document either oral fluid or urine drug test results, including sample validity,
- Record the Collector’s identification conducting the test,
- Record the donor’s consent for the test
- Use as chain of custody form for you confirmatory laboratory, and
- creates a carbon copy for the donor and collector.
The triplicate copies allow you to give one copy to the laboratory with any confirmation samples, keep one copy for your own records, and give a copy to the donor for their records. You can be assured that each Form contains all the necessary fields, recording all the right information.
